Quase um quarto dos casos de câncer de pênis no Brasil leva à amputação.

SAMUEL FERNANDES (FOLHAPRESS) – O Brasil registrou, de janeiro a novembro do ano passado, 1.933 casos de cânceres de pênis. Desses, cerca de um quarto precisou ser retirado por causa do tumor. Os dados foram compilados pela Sociedade Brasileira de Urologia (SBU) com base em informações do DataSUS.

Para Alfredo Canalini, presidente da entidade, o total de casos é preocupante. “O Brasil tem números que são assustadoramente altos. Eles só são superados por países da África subsaariana.”

Os diagnósticos não são muito diferentes do que foi observado em outros anos. De 2018 a 2022, a média anual de casos desse tipo de tumor foi de 2.071.

Embora o número não apresente queda, a diminuição nos casos da doença não é uma tarefa tão difícil. Isso porque uma das principais formas de evitar o tumor é a higienização diária e adequada do órgão.

Canalini explica que os homens precisam puxar o prepúcio, pele que envolve a glande, que é a cabeça do pênis. Então, lavar com água e sabão a região de forma a retirar toda a sujeira que pode se acumular. Após, é importante secar.

Caso o homem não consiga puxar completamente o prepúcio, é um caso de fimose. Com isso, o homem precisa procurar um médico a fim de seja efetuado um procedimento que permita a limpeza adequada do pênis.

O problema é que essa simples informação de como limpar o órgão ainda não é de conhecimento de boa parte dos brasileiros. “Nós precisamos disponibilizar informação para essas pessoas, porque aprender a lavar o pênis e usar um sabonete é uma coisa básica, mas é necessário aprender”, afirma Canalini.

Além da limpeza, a vacina contra o HPV (vírus do papiloma humano) é outro mecanismo útil para evitar o tumor na genitália. O SUS disponibiliza o imunizante para meninos e meninas de 9 a 14 anos. No entanto, Canalini afirma que os meninos tendem a desconhecer o imunizante, o que representa um problema na prevenção do câncer de pênis.

Em casos mais graves do tumor, a amputação do pênis é necessária.

Segundo os dados preliminares levantados pela SBU, foram 459 registradas em 2022. Esse é o menor valor desde 2018, quando foram 639 remoções da genitália. Mesmo assim, o número é alto: considerando o total de casos em 2022, quase um quarto dos diagnósticos resultou na retirada do órgão.

Esse fato lembra outro desafio: o diagnóstico precoce. A retirada completa da genitália só é necessária quando o tumor se encontra em um estágio muito avançado. Em momentos iniciais, procedimentos mais simples no local podem ser adotados para curar o paciente. Outra saída, em um momento intermediário da doença, é uma amputação parcial, em que somente uma porção do órgão é removida.

SINTOMA MAIS COMUM

Um diagnóstico precoce envolve uma rotina comum de ida ao médico e também estar atento a alguns sinais que podem ser um indicativo do tumor. Canalini acrescenta que, no início, o sintoma mais comum é o surgimento de uma lesão na glande ou na parte interna do prepúcio. Pode ser uma verruga, por exemplo.

Com o tempo, a ferida tende a ficar cada vez mais complicada. Isso é um sinal de que a doença está avançando. Queimação na região também é um relato comum, além de um cheiro forte.

A limpeza, além de prevenir, também é útil para a descoberta precoce da doença, porque o homem que realiza a higiene adequada tende a observar de maneira rotineira a genitália.

E mesmo que não seja o tumor, estar atento ao órgão é importante porque pode ser outra condição. A sífilis, por exemplo, tem como um de seus sintomas, ainda na fase primária, o aparecimento de uma lesão na genitália.

Mas a amputação do pênis, embora para alguns homens seja visto como o pior cenário no mundo, não é o desfecho mais trágico. Existem casos em que a doença se espalha para outras regiões do corpo. Nesse caso, os riscos de morte aumentam.

Em 2020, último ano com informações disponíveis de mortalidade por câncer de pênis, foram 463 fatalidades pelo tumor, o que representa cerca de 22% do total de casos daquele ano.
